Integrated Medication Management in the Care of Geriatric Patients with Congestive Heart Failure Using the MINE Application (Medicine IN gEriatric) at A.W. Sjahranie Regional General Hospital, Samarinda
Congestive Heart Failure (CHF) is a chronic condition with high morbidity and mortality, especially among older adults. This study assessed the effectiveness of the Integrated Medicine Management (IMM) model, which emphasizes collaboration between pharmacists, physicians, nurses, and patients, in minimizing drug-related problems (DRPs) and improving clinical outcomes in hospitalized elderly patients with CHF at RSUD A. W. Sjahranie Samarinda. The research was conducted in three phases: (1) an analysis of standard operating procedures (SOPs) to assess gaps in hospital service implementation, (2) expert validation of the IMM model, and (3) dissemination of the model through the MINE (Medicine in Geriatric) application, followed by an experimental study. The patients were divided into control and intervention groups, with the latter receiving IMM-based pharmaceutical care. The IMM intervention significantly reduced the use of Potentially Inappropriate Medications (PIM), shortened hospital stays, lowered readmission rates, and improved quality of life based on EQ5D-5L VAS assessments. This study underscores the critical role of interdisciplinary collaboration and structured discharge planning, including medication reconciliation and patient education, in optimizing treatment and enhancing the safety of elderly patients with CHF. The results advocate for the broader adoption of the IMM model in clinical pharmacy services, particularly for managing chronic diseases in geriatric populations, to improve therapeutic outcomes and reduce healthcare burdens.
